Melissa Taylor: Consent for High Performance Coaching

Consent:

I consent to high performance executive and life coaching provided by Melissa Taylor, M.A. of Salt + Light Coaching, LLC; which involves utilizing Neurolinguistic Programming, positive psychology, guided mental exercises, and Time Line TherapyTM methods. I understand that I am fully in control of my state, that I cannot be forced to believe or behave in any way that I do not wish, and that I can disengage from a coaching process at any time. 

I agree to receive communication regarding appointments, events, tips, and promotions via email and text communication. I understand I can opt-out at any time by clicking "unsubscribe" in emails and replying with "stop" in text.

I understand that the services rendered by Melissa Taylor or anyone representing Salt + Light Coaching, LLC; hereby known as Coach, are non-therapeutic coaching exercises, to develop self-awareness, positive thinking, and personal development for more increased choice in habits, behavior, and mindset. I also understand that Coach’s services are for educational purposes only, and that Coach does not represent her services as any form of health care or psychotherapy. Despite research to the contrary, by law, Coach makes no health benefit claims for her services.

Further, I agree that high performance mindset coaching provides tools for self-improvement and is not a replacement for medical treatment or psychiatric care; thus should be considered as supplemental and complimentary to such conventional treatments. I understand that Coach does not guarantee successful results and the effectiveness of coaching varies by individual. Any decisions I make having received any services by Coach are my own and I remain wholly responsible for any decisions or actions that I take.

Confidentiality
I understand that Coach respects my privacy. All information disclosed to Melissa Taylor or anyone associated with Salt and Light Coaching, LLC in regards to my client file will be kept fully confidential.

Payments and Cancellations

I agree to pay for services rendered by Coach in full up to 24 hours prior to my appointment; unless discussed otherwise. I also understand that any appointment scheduled with me is reserved specifically for me. Therefore, I agree to pay the full amount of any session booked that I do not cancel by 24 hours prior to my appointment.

Assignments: I also understand that my commitment is required in order to obtain maximum results. Therefore, I agree that if I do not complete my required action steps assigned to me by Coach, and turn in at the beginning of each session, then I may be asked to forfeit my session, without a refund, and reschedule when I can commit to the required tasks.

Workshops and Special Events

I agree to pay for workshops and special events prior to, or at time of event, unless otherwise agreed upon in writing with Melissa Taylor. I understand that my registration for any event reserves a space specifically for me. Therefore, I agree to notify Coach within seven days of the start of the event or series if I am not able to attend. I also understand that I will not receive any refund should any cancellation be made less than seven days from the start of, or during the duration of, an event or workshop series.
